Referrals

Referrals for a driving assessment can be initiated by a client, family member, GP, Support Coordinator / Support Planner, GP/specialist, allied health professional, client or family member. All referrals must be accompanied by medical clearance and certificate from a GP/specialist, and a vision assessment completed by an Optometrist/Ophthalmologis.

  • NIISQ participants
  • TAC clients
  • WorkCover
  • iCare
  • other general insurance / solicitor based clients
Clients:

Driving Well Occupational Therapy targets the following client groups:

  • Older clients with general frailty/ageing, physical/functional problems, cognitive decline, dementia etc
  • Clients with chronic health conditions eg diabetes
  • Clients with neurological conditions eg stroke, parkinsons, spinal cord injury
  • Clients with orthopaedic injuries eg amputations, hand and shoulder injuries, back injuries
  • Younger/learner drivers with physical impairments such as cerebral palsy, spinal cord injury
  • Younger/learner drivers with learning difficulties eg Aspergers and Autism

Refer to the AustRoads Assessing Fitness to Drive Guidelines regarding decision making on fitness to drive, and need for conditional licence and OT driving Assessment.

Due to complexity and demands, we do not accept referrals for clients with mental health diagnoses.

Client Referrals

Refer yourself/ your client through our “Client Referral Form” and fill out your / your client’s details and reason for the referral. Start uploading relevant documentation. You can “save” the form and return too the form any time to add the rest of your documents. Zeta our Practice Manager will ring you to book an appointment.


Please note: due to complexity, our clinicians skills set, and demands – we do not see clients with mental health conditions.

Referrals Must Include:

1. Referral Form and Clearance to undergo OT Driving Assessment signed by GP/Specialist

2. QLD Transport Medical Certificate

3. Vision Assessment Form
Older drivers, clients with neurological conditions/injuries MUST see the Optometrist / Ophthalmologist to complete this form. Otherwise GP can do visual acuity and visual fields on the QLD Transport Medical Certificate.

4. NDIS Plan:

  • Copy of NDIS plan
  • Full name
  • Who is the decision maker for the client
  • DOB
  • Address
  • Phone number
  • Email address
  • Purpose of assessment
  • Client’s medical condition/disability
  • Current licence class (none vs learner vs open)
  • Basic overview of current level of function

5. Hospital discharge summary or any reports previously completed by community / paediatric Occupational Therapists, Psychologists, Neurologist, Psychiatrists, Physiotherapist.

6. Photograph of Driver’s licence.
​Withholding information can result in an inaccurate assessment and incorrect recommendations – which could lead to significant risks for the client and the community.
